$77,500 - $87,500 (based on experience) Join Our Team as a Wound Care Nurse (RN/LPN) Lake City Healthcare and Rehabilitation is seeking a skilled and compassionate Wound Care Nurse (RN/LPN) to join our clinical team. In this essential role, you will work collaboratively with the interdisciplinary team (IDT) to provide high-quality wound and skin care, promoting healing, comfort, and overall well-being for our residents. Key Responsibilities Collaborate with the interdisciplinary team (IDT) to develop, implement, and evaluate individualized care plans Complete applicable sections of the Minimum Data Set (MDS) accurately and timely Assess, treat, and manage both primary and secondary wounds Perform routine skin assessments and monitor for changes in condition Identify residents at risk for pressure ulcers and implement preventive measures Assist with pain management related to wound care Ensure compliance with current wound care policies, procedures, and best practices Educate residents and families on wound care treatments and plans in a clear and compassionate manner Evaluate and support nutritional needs that impact wound healing Recommend appropriate wound care treatments, dressings, and pressure-relieving devices Promptly report any concerns of abuse, neglect, or safety issues in accordance with facility policies Qualifications Active, unencumbered RN or LPN license in the state of Florida Minimum of 2 years of nursing experience; wound care experience preferred Strong clinical assessment, documentation, and communication skills Experience in skilled nursing or long-term care is a plus Why Join Us?
