The BEST work with US! The Role The home health nurses will be responsible for providing high quality and coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives. What You Will Do Conduct comprehensive patient assessments in the home setting to evaluate the patient's physical and emotional health. Develop and implement individualized care plans based on physician orders, patient goals, and clinical best practices. Administer medications, injections, wound care, and other treatments as prescribed by the physician Provide patient and caregiver education on disease management, medication usage, home safety, and recovery techniques. Coordinate care with other healthcare providers, including physicians and therapists, to ensure continuity of care. Requirements Current and unrestricted RN license in the state of Florida. Strong clinical skills and ability to perform evidenced based nursing practices. 1+ years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health. Excellent communication and interpersonal skills with the ability to work effectively and independently with patients, families, and healthcare providers.
