POSITION SUMMARY
The RN Liaison/Admission Nurse serves as a key clinical liaison between Gulfside Hospice, patients and families, physicians, hospitals, facilities, case managers, social workers, and other referral partners. The position facilitates hospice referrals, develops and maintains professional relationships with referral partners, evaluates patients for hospice appropriateness, and completes admissions when appropriate. The RN provides timely, individualized, and accurate clinical assessments while ensuring compliance with hospice admission criteria, organizational standards, and applicable regulations.EDUCATION AND QUALIFICATIONS
Licensed in the State of Florida as a Registered Nurse. Minimum of two (2) years clinical experience in Hospice or related nursing experience. AHA approved CPR certification. Must possess and maintain Florida driver's license. Must possess and maintain automobile insurance.ESSENTIAL JOB RESPONSIBILITIES
Serve as a professional liaison between Gulfside Hospice and referral partners, including hospitals, facilities, physicians, case managers, and social workers. Build and maintain positive professional relationships with referral partners and serves as a clinical resource regarding hospice services and admission criteria. Facilitate hospice referrals and assists referral partners with appropriate hospice services and alternatives when patients do not meet hospice criteria. Perform timely, individualized clinical assessments of patients referred for hospice services and evaluate appropriateness based on established hospice admission criteria and LCD requirements. Meet with patients and families to explain hospice services, address individual needs and concerns, manage expectations, and obtain required consent documentation. Communicate effectively with patients, families, physicians, supervisors, and referral partners to facilitate a smooth admission process. Initiate the patient's plan of care and provide education regarding hospice services. Complete required admission documentation accurately and within established timeframes. Provide assessment of the patient and/or family's physical, psychological, social, and spiritual needs and initiate appropriate interventions. Anticipate patient needs and coordinate appropriate DME, medications, supplies, transportation, nursing, CNA, and volunteer services as needed for admission. Serve as a hospice resource to hospital and community partners and assists with questions regarding hospice services, policies, and admission requirements. Address patient and family concerns and assist with timely resolution of unmet expectations. Maintain awareness of facility changes, programs, procedures, and other information that may affect hospice referrals and services. Work collaboratively with the Patient Access Team and other interdisciplinary team members to coordinate patient services and appropriate community resources. Maintain professional standards of hospice nursing practice and ensure patient care and documentation comply with organizational, state, and federal requirements. Demonstrate strong clinical judgment, critical thinking, communication, organization, adaptability, and the ability to manage multiple priorities. Proficiently utilize the Electronic Medical Record and maintain accurate and timely documentation. Participate in rotating holiday coverage as assigned. Performs other duties as assigned.SKILLS AND COMPETENCIES

Ensure compliance with all patient care and meet documentation standards and regulations. Ability to prioritize and manage multiple referrals and admissions in a fast-paced environment. Knowledge of hospice services, admission criteria, and regulatory requirements. Proficient in Electronic Medical Records.
