Home health visits in North Hillsborough & East Pasco counties (including North Tampa, Lutz, New Tampa, Tampa Palms, Wesley Chapel, Land O' Lakes, Dade City, San Antonio, and surrounding areas).
Schedule:
This is a part-time Weekend Baylor role requiring full weekends . Saturday and Sunday (20 hours per week). What You'll Do Perform comprehensive patient assessments, including OASIS assessments at required intervals Develop, initiate, and revise individualized plans of care as patient needs evolve Deliver skilled nursing services and treatments as outlined in the plan of care Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies Collaborate with the interdisciplinary team to ensure seamless, holistic care Why Join Empath Health?
