Job Description
IN-PERSON, OFFICE-BASED POSITION
Nayar Health Care is seeking an experienced Registered Nurse to join our Quality Assurance team. This is a 100% in-person, office-based position and is NOT remote or hybrid. The RN will work from our office and focus on reviewing clinical documentation and Plans of Care to ensure accuracy, completeness, compliance, and quality across our Home Health services. What You'll Do Review SOC, ROC, Recertification, Transfer, and Discharge documentation Review and finalize Plans of Care (POCs) Ensure documentation supports medical necessity and homebound status Review physician orders, diagnoses, goals, interventions, and visit frequencies for accuracy and consistency Identify documentation deficiencies and communicate necessary corrections to clinical staff Ensure documentation is complete and compliant prior to finalization Work closely with DONs, clinical managers, and field clinicians to resolve documentation concerns Assist with QA/QAPI, compliance, and survey readiness Support compliance with CMS, Medicare, and ACHC requirements What We're Looking For Active RN license Home Health experience strongly preferred Strong understanding of Plans of Care and Home Health documentation OASIS knowledge and experience preferred Familiarity with Medicare/CMS requirements Excellent attention to detail and strong clinical judgment Ability to communicate effectively with clinical staff Must be able and willing to work in the office full-time — this position is not remote Why Join Nayar Health Care?
