Job Overview A Home Health Field Registered Nurse who is in the case manager of the Home Health Team is responsible for the nursing care of the patients assigned to them and directs the Home Health Aides in quality patient care. The Field Registered Nurse is responsible for assessing patient and family needs in order to promote the best care the Horne Health can give for recovery and rehabilitation. Provide nursing services within the scope of practice authorized by the license issued by the State for a registered nurse. As a vital member of our home health team, you will coordinate patient assessments, deliver personalized treatments, and educate families to promote optimal health outcomes. Duties Conduct thorough patient assessments including vital signs, physical examinations, and detailed medical histories to determine individualized care plans. Administer medications and injections, including IV infusions and immunizations, following strict aseptic techniques and safety protocols. Manage complex wound care procedures such as suturing, ostomy care, and dressing changes while adhering to infection control standards. Perform specialized treatments including catheterization, tube feeding management, ventilator support, and dialysis procedures. Utilize electronic health record (EHR) systems like for accurate documentation of patient care activities and medical records. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of services across acute care hospitals, nursing homes, assisted living facilities, and community clinics. Provide patient education on medication administration, disease management—including cardiac conditions, infectious diseases, Alzheimer's/dementia care—and health coaching tailored to individual needs. Participate in triage calls and urgent visits to evaluate patient conditions promptly and determine appropriate interventions or referrals. Maintain compliance with HIPAA regulations and NCQA standards while ensuring the confidentiality and security of all patient information. Support patients with developmental disabilities or behavioral health needs by applying specialized nursing techniques and compassionate communication strategies. Qualifications A graduate of an accredited School of Nursing and be licensed in the state of Florida. Complete knowledge of nursing principles and procedures of skills in the technique of good patient care. Good mental health and the quality of retaining emotional stability in situations of varying circumstances. Recommend experience in diverse clinical environments such as hospital medicine, ICU, emergency medicine, or post-acute care settings like nursing homes or hospice. Knowledge of advanced nursing procedures including IV insertion/infusion therapy, laboratory specimen collection/processing, phlebotomy, and diagnostic evaluation techniques. Excellent assessment skills. Ability to work independently in home settings while maintaining high standards of safety and professionalism. Minimum of one year experience, preferable in community health, not required. Meets the physical and health requirements of the Home Health. Skills and experience in providing high-quality Patient Care and Personal Care Strong Communication skills, including interpersonal and active listening abilities Compassionate, empathetic nature with a commitment to client well-being BLS/CPR certification.
