As a Nurse Practitioner in this per diem role, you will complete thorough in-home evaluations in locations including Tallahassee, Woodville, and Midway. Each assessment typically takes about 45 minutes and could range from annual health assessments to post-discharge visits, helping to bridge critical care gaps in the community.

Key Responsibilities:

Complete comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patients current and previous medical history, conditions, and medication use Educate patients on their conditions and prescribed medications Identify gaps in care, providing recommendations to enhance patient health and safety No requirements for treatment plan development or medication prescribing

Qualifications:
