Job Summary We are seeking a dedicated and compassionate Registered Nurse to join our dynamic healthcare team. Duties Perform comprehensive OASIS assessments, including vital signs, and medical history documentation using EMR systems such as Well Sky. Administer treatments ordered by a physician; wound care, or IV infusion, ensuring adherence to aseptic technique and safety protocols. Document all patient interactions meticulously in medical records while maintaining HIPAA compliance. Collaborate with multidisciplinary teams for positive outcome measures with discharge planning, and clinical manager guidance to ensure patient goal centered care is met prior to discharge. Experience working in the home health environment is a plus. Familiarity with specialized patient care needs such as gastrostomy tube feeding, and Foley catheter care using teach and train techniques in patient's home. Ability to perform assessments for patient needs effectively while demonstrating excellent critical thinking skills. Knowledge of infection control practices and aseptic techniques essential for maintaining a safe environment. Strong communication skills to educate patients on health coaching topics like fall risks, home safety, nutrition or medication adherence.
