Position Summary We are seeking an experienced and dependable Registered Nurse (RN) to join our home health team. This fulltime position offers the opportunity to provide skilled nursing services to patients in their homes while maintaining flexible hours that fit your lifestyle both on campus of our senior living community and surrounding Lake County. Responsibilities Perform comprehensive nursing assessments and develop individualized care plans. Provide skilled nursing services in accordance with physician orders, Medicare guidelines, and agency policies. Administer medications, treatments, and wound care as prescribed. Educate patients and caregivers on disease processes, medications, and home care techniques. Communicate effectively with physicians, case managers, and other members of the care team. Accurately document all care and communication in the Axxess EMR system. Qualifications Current and active Registered Nurse (RN) license in the State of Florida Minimum of one (1) year of clinical experience (home health experience preferred). Knowledge of Medicare home health regulations and OASIS documentation. Current CPR certification. Valid driver's license, reliable transportation, and proof of insurance.
