  • Licensed Practical Nurse (LPN) to join our team in Lee County.
The LPN Case Manager is responsible for the provision of services that assist patients with the achievement of their highest level of wellness. S/he is focused on client centered services that link clients and other family members with health care, psychosocial and other home and community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care. S/he is responsible for, coordination, advocacy, education, monitoring and accurate, timely documentation in all program areas. S/he demonstrates knowledge of growth and development and possesses the ability to interact, communicate and care for age specific patient populations. ‎ How will you make an impact & Requirements ‎ What You'll Do Maintain relationship with hospitals and facilities Ensure all documents are completed timely and accurately for patients Keep an active log of all admitted patients in hospitals and facilities Participate in a weekly IDT meeting and quarterly summit meetings Follow up on the discharge of the patient to and from hospitals and facilities back to home. Collaborate with third party agencies as needed. Other duties may be assigned. Qualifications Education and/or Experience Licensed Practical Nurse 2 or more years of experience in business operations in a clinical setting Clinical knowledge with Medicare populations Strong project management skills Ability to communicate effectively and demonstration in identifying problems and effectively relay insights to facilitate corrections and improvements Strong ability to influence others with demonstrated self-discipline Adept in cross functional interactions and collaboration and meeting program goals Strong computer skills in MS Office Suite
  • PowerPoint, Excel, Word Language Skills
  • Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Mathematical Skills
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Reasoning Ability
  • Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Computer Skills
  • To perform this job successfully, an individual should have knowledge of Athena Database software; Microsoft Spreadsheet software and Microsoft Word Processing software; including Excel.

Compensation Range:
$22.99 to $34.49 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
