Take on a rewarding opportunity delivering skilled nursing care directly to patients in their homes. This full-time, 13-week contract assignment provides a structured weekday schedule and the fulfillment of helping individuals maintain independence and health in their own surroundings.

Schedule & Setting:

Monday Friday, 8:00 AM 5:00

PM Field-based:

daily travel to patient homes within the assigned territory (mileage reimbursement provided)

Caseload:

30 points per week (typically 35 home visits daily) Utilize Home Care Home Base (HCHB) for electronic documentation and OASIS for assessments

Qualifications:

Active Registered Nurse license in the state of practice, in good standing At least 1 year of recent Home Health nursing experience; field experience preferred Proficiency with Home Care Home Base (HCHB) and OASIS documentation Reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

Perform thorough OASIS assessments and create individualized patient care plans Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management Monitor and evaluate patient progress, identifying and communicating changes in condition to care teams and physicians Educate patients and families on disease management, treatment options, and home care best practices Accurately document all care provided, and maintain regulatory compliance Collaborate with the broader home health team, including therapy, social work, and aide staff Participate in interdisciplinary case conferences, team meetings, and continuing education opportunities
