Position Summary A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing Key Responsibilities Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs. Provides those services requiring substantial specialized nursing skills. Initiates appropriate preventive and rehabilitative nursing procedures. Determines the amount and type of nursing needed by each individual patient, in conjunction with the team members, physician and patient or patient representative. Gives total patient care as needed. Counsels the patient and family/significant others in meeting nursing and related needs. Initiates the plan of care and necessary revisions. Complete clinical and progress notes for each patient visit and summaries after his/her patients in a timely manner Informs personnel of changes in the condition and needs of the patient. Timely submission of orders notifies physician of patient needs Completes treatment, certification/recertification orders and remove transfer/discharge summaries. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames. Appropriately utilizes ICD-10 codes. Provides services in accordance with the plan of care. Understands and adheres to established Agency policies and procedures. Participate in in-service programs. Participates in peer review and performance improvement as assigned. Collaborate care with clinical manager, MD and all assigned disciplines associated with patient care. Participates in utilization review of medical records as assigned, and QAPI committee as assigned. Be available for after hours, weekends and holiday visits as assigned. Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills. Supervises and teaches other nursing personnel. Coordinates services. Conducts patient care conferences on patients assigned to his/her care. Available for ongoing assignments and timely responsive to office personnel correspondence Ensure supplies and required items are available for patient care at all times. Perform other duties and responsibilities as assigned.
Qualifications Education & Experience:
Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing. Two (2) years nursing experience, preferred. Medicare Experience, EMR experience (preferably Axxess) Home Health, Wound Care, Infusion, OASIS experience a must. Acceptance of philosophy and goals of this Agency. Ability to exercise initiative and independent judgement.
Skills & Competencies:
Excellent leadership, communication, and negotiation skills Strategic thinking and execution skills Excellent computer skills, proficient with Axxess.com Creative problem-solving skills, organized and detail oriented. OASIS assessment experience required. Flexible and able to multi-task, time management skills Team player, positive attitude, proactive and consistent follow through Capable of working independently and exercising discretion and independent judgment Ability to participate in physical activity Ability to work for extended period of time while standing and being involved in physical activity. Heavy lifting. Ability to do extensive bending, lifting, and standing on a regular basis.
Work Environment:
This position requires travel to client homes, participation in community marketing events, and presence in office when not participating in off-site activities.
