Clinical Team Leader / Quality Assurance Nurse Legacy Home Health Care is seeking an experienced and detail-oriented Clinical Team Leader / Quality Assurance professional to support our clinical operations and help ensure the delivery of exceptional, compliant patient care. This position is ideal for a collaborative clinical leader who understands the unique demands of home health, enjoys developing strong teams, and has an exceptional eye for accuracy and quality. Key Responsibilities Provide leadership, guidance, and support to clinical team members. Promote teamwork, accountability, and effective communication across departments. Review clinical documentation for accuracy, completeness, consistency, and regulatory compliance. Identify documentation concerns and provide timely, constructive feedback to clinicians. Monitor quality indicators and assist with performance-improvement initiatives. Help ensure compliance with Medicare Conditions of Participation, agency policies, and applicable state and federal regulations. Support clinicians with care coordination, documentation standards, and clinical best practices. Assist with employee training, coaching, performance management, and professional development. Collaborate with agency leadership to improve clinical outcomes, operational efficiency, and patient satisfaction. Help foster a positive workplace culture centered on teamwork, quality, and excellent patient care. Qualifications Previous home health experience is required. Current, unrestricted RN license. Prior leadership, supervisory, management, or quality assurance experience preferred. Strong knowledge of home health documentation and regulatory requirements. Exceptional attention to detail and commitment to accuracy. Demonstrated ability to lead, coach, and motivate a clinical team. Strong communication, organization, and problem-solving skills. Ability to manage multiple priorities and meet deadlines in a fast-paced environment. Experience with electronic medical record systems and clinical chart review preferred.
