Registered Nurse Job Summary:
Viventium Home Care Agency is seeking a compassionate and skilled Registered Nurse to join our team in New York, New York. As a dedicated home care provider, we pride ourselves on delivering high-quality patient care and support to those in need. We are excited to welcome a talented and motivated Registered Nurse to our team, who will make a meaningful impact in the lives of our patients.
Responsibilities:
Provide direct patient care to individuals in their homes, focusing on promoting health, preventing illness, and alleviating suffering Conduct initial and ongoing assessments of patients' physical, emotional, and social needs Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams Administer medications, treatments, and therapies as prescribed by physicians and medical directors Collaborate with interdisciplinary teams to ensure comprehensive care and continuity of services Maintain accurate and timely documentation of patient care and services rendered Educate patients and families on disease management, self-care, and health promotion strategies Participate in quality improvement initiatives to enhance patient outcomes and agency performance
Requirements:
Current New York State Registered Nurse license BLS (Basic Life Support) certification Ability to lift up to 50 pounds and perform physical tasks as required Excellent communication and interpersonal skills
