Take your nursing expertise into the community with this full-time, 13-week contract opportunity specializing in home health care. As a registered nurse, youll provide vital care to patients in their homes, offering both clinical skill and compassionate support throughout Winter Garden, FL.

PM Key Duties:

Manage a weekly caseload of 30 points (typically 35 patient visits per day). Travel daily to patient homes within your assigned territory, with mileage reimbursement. Complete OASIS assessments to develop personalized care plans. Deliver skilled nursing care, including wound care, IV therapy, medication management, and chronic disease education. Continually assess patient progress, recognizing changes in condition and updating the care team promptly. Educate patients and families regarding disease management, medication protocols, and preventative strategies. Document accurately and promptly in Home Care Home Base (HCHB). Work collaboratively with an interdisciplinary team, attending case conferences and contributing to coordinated care. Uphold all documentation, regulatory, and agency standards in patient care.

Qualifications:

Active RN license in good standing for the state. At least one year of recent home health experience; field work strongly preferred. Confident using Home Care Home Base (HCHB) and experienced with OASIS. Possess reliable transportation, a valid drivers license, and auto insurance.
