What You'll Do Provide skilled nursing care in patients' homes according to physician orders and established plans of care Complete comprehensive patient assessments and ongoing evaluations Develop, implement, and update individualized plans of care Educate patients and caregivers regarding medications, disease management, treatments, and safety Monitor patient progress and communicate changes in condition Coordinate care with physicians, therapists, and other members of the interdisciplinary team Complete timely and accurate clinical documentation Participate in case conferences and care coordination Follow agency policies and applicable regulatory requirements What We're Looking For Current and active Florida Registered Nurse (RN) license Home health experience preferred, but not required Strong assessment and clinical skills Excellent communication and organizational abilities Ability to work independently and manage a flexible schedule Current CPR certification Reliable transportation and valid Florida driver's license Must be able to successfully complete a Level 2 background screening with fingerprinting in accordance with Florida law and Agency for Health Care Administration (AHCA) requirements Never Worked in Home Health Before?
