Qualifications:

Active Nurse Practitioner license in Georgia Current certification in CPR (BLS) Board certification through AANP or ANCC At least six months of recent Nurse Practitioner experience preferred Master of Science in Nursing from an accredited institution

Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review prior and current medical histories, ongoing conditions, and medication regimens Educate patients about medical conditions and appropriate medication use Detect gaps in care and suggest actionable recommendations Documentation and assessments required; no prescribing or treatment plans necessary
