Job Description

Job Overview We are seeking a compassionate and highly skilled Hospice Registered Nurse Case Manager to join our dedicated healthcare team. In this vital role, you will coordinate and deliver comprehensive hospice care, ensuring patients and their families receive the highest quality support during challenging times. Your expertise in patient assessment, clinical case management, and interdisciplinary collaboration will help promote comfort, dignity, and peace for those in end-of-life care. Duties Conduct thorough patient assessments to determine care needs, including vital signs, physiological status, and symptom management. Develop personalized care plans in collaboration with physicians, social workers, chaplains, and other healthcare professionals to meet each patient's unique needs. Manage clinical case documentation accurately using Electronic Health Records (EHR) systems while ensuring compliance with HIPAA regulations. Provide direct patient care in various settings such as home health visits, inpatient facilities, or outpatient clinics, emphasizing comfort and symptom control. Coordinate discharge planning and post-acute care arrangements to ensure seamless transitions between care settings. Educate patients and families about disease processes, medication management, and available hospice resources to support informed decision-making. Monitor patient progress regularly through diagnostic evaluation and adjust care plans accordingly to optimize quality of life. Collaborate with healthcare professionals across disciplines to facilitate effective utilization management and ensure adherence to health regulation policies. Engage in patient service activities such as intake assessments, managing symptoms related to infectious diseases or trauma, and supporting behavioral health needs. Experience Proven experience as a Registered Nurse with a strong background in hospice or palliative care. Critical care experience including ICU, Emergency Room (ER), or Level I/II Trauma Center settings is highly desirable. Demonstrated expertise in medical-surgical nursing, hospital medicine, or home health environments. Familiarity with medical terminology, physiology knowledge, and clinical case management practices essential for effective patient assessment and treatment planning. Experience managing electronic health records (EHR/EHR) systems with proficiency in documentation standards such as HIPAA compliance. Knowledge of health regulation policies related to hospice care and post-acute services is preferred. Ability to collaborate effectively with multidisciplinary teams including physicians, social workers, behavioral health specialists, and other healthcare providers. Strong communication skills for client interaction and family education across diverse populations including geriatrics and pediatrics when applicable. Prior inpatient or outpatient experience involving diagnostic evaluation, discharge planning, or managed care is advantageous. $60,000.00 - $75,000.00 per year
