Key Responsibilities:

Conduct comprehensive annual risk assessments for Medicare Advantage members in their homes Evaluate both past and current medical histories, active medical conditions, and medication regimens Educate patients on their health conditions and medication use Identify care gaps and provide informed recommendations to improve patient outcomes No responsibility for creating treatment plans or prescribing medications

Qualifications:

Nurse Practitioner license valid in Georgia Active BLS/CPR certification Board certified through AANP or ANCC Minimum six months recent experience as a Nurse Practitioner preferred MSN from an accredited nursing program This position covers Douglasville, Villa Rica, Austell, and surrounding Douglas County area members.
