Duties Perform comprehensive patient assessments to determine suitability for home dialysis therapies, including peritoneal dialysis and hemodialysis. Educate patients and their caregivers on dialysis procedures, infection control practices, and equipment maintenance to promote independence and safety. Set up, operate, troubleshoot, and maintain dialysis equipment in accordance with clinical protocols and manufacturer guidelines. Monitor patient vital signs, fluid status, and treatment responses during dialysis sessions to ensure optimal outcomes. Collaborate with interdisciplinary teams—including nephrologists, social workers, and dietitians—to develop individualized care plans. Document all patient interactions, treatments, and observations accurately within electronic health record (EHR) systems such as Epic or Cerner. Provide emergency response when necessary, including managing acute pain or complications related to dialysis or comorbid conditions like infectious disease or cardiovascular issues. Requirements Valid RN license with current state certification; experience in nephrology or dialysis preferred but not required. Experience with Peritoneal Dialysis or Home Hemodialysis equipment and training. Proficiency with electronic medical records (EMR/EHR systems) Knowledge of aseptic technique, infection control standards, and sterile processing procedures. Strong assessment skills with the ability to recognize early signs of complications such as infection or fluid imbalance. Excellent communication skills for patient education and interdisciplinary collaboration. Ability to work independently in a home setting while maintaining high standards of safety and professionalism.
