Job Description

An excellent opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN). This role involves delivering skilled nursing care to patients within their homes, ensuring each individual receives personalized, high-quality support. The ideal candidate is motivated, organized, and proficient in fostering positive patient outcomes as part of an interdisciplinary team.
Schedule:
Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Field-based, with daily travel to patient homes (mileage reimbursement included)
Caseload:
2532 points per week (typically 35 patient visits per day)
Key Responsibilities:
Perform thorough OASIS assessments and develop individualized care plans for patients. Deliver skilled nursing care, including wound management, IV therapy, medication administration, and chronic disease management. Monitor progress, identify changes in condition, and coordinate promptly with care teams and physicians. Provide thorough education to patients and families regarding treatment plans, preventive measures, and disease management. Complete accurate documentation in Home Care Home Base (HCHB) and participate in case conferences and ongoing education. Ensure care aligns with all regulatory and agency standards.
Qualifications:
Active RN license (unrestricted, state of practice) At least 1 year of recent Home Health RN experience in the field Proficiency with Home Care Home Base (HCHB) and OASIS essential Strong clinical assessment, organizational, and communication skills Reliable transportation, valid drivers license, and current auto insurance Ability to work independently and manage a flexible field-based workload
Preferred:
BSN degree Experience managing a caseload of 2532 points per week Understanding of Medicare and home health compliance requirements
