Position Overview:

We are seeking a highly skilled and compassionate Supervisory Home Health Care Nurse to join our team on a contract or retainer basis. This role is ideal for a Registered Nurse (RN) looking to supplement their income with flexible hours. The nurse will conduct in-person patient assessments biweekly in the Hazlehurst area, dedicating 2-4 hours per session.

Key Responsibilities:

Conduct comprehensive in-person patient assessments in compliance with GAPP standards. Evaluate and document the patient's health status, care plan effectiveness, and caregiver performance. Provide guidance to caregivers on improving patient outcomes and adhering to care protocols. Ensure all documentation meets Medicaid and GAPP regulatory requirements. Collaborate with the care team to address any issues or concerns that arise during assessments. Maintain clear communication with the agency and caregivers to ensure continuity of care.

Schedule:

Biweekly patient assessments, with each session ranging from 2-4 hours. Flexible scheduling to accommodate both the nurse's and patients' availability.

Qualifications:

Active Registered Nurse (RN) license in the state of Georgia. Minimum of 2 years of nursing experience, preferably in pediatric or home healthcare. Familiarity with GAPP standards and Medicaid requirements is highly preferred. Strong organizational and communication skills. Ability to work independently and maintain professional boundaries.
