In this contract opportunity, youll provide essential skilled nursing services in a field-based role, supporting patients within their homes and ensuring they experience seamless, patient-focused care throughout their recovery journey.

Schedule/Hours:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00

PM Caseload:

2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

Perform comprehensive patient assessments using OASIS and create individualized care plans. Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management. Monitor and document patient progress accurately in Home Care Home Base (HCHB). Educate patients and families regarding treatment plans, medications, and ongoing care needs. Communicate promptly with interdisciplinary team members and physicians about changes in patient status. Participate in staff meetings, professional development, and case conferences as required. Ensure regulatory compliance and uphold high standards of clinical documentation and patient care. Travel daily to patient homes in the assigned area (mileage reimbursement is provided).

Required Qualifications:

Active RN License in good standing (state-specific) Minimum 1 year recent home health experience (field experience strongly preferred) Proficiency in Home Care Home Base (HCHB) and OASIS is required Reliable transportation, valid drivers license, and auto insurance Exceptional clinical assessment, communication, and organizational skills Independent work ethic and adaptability to a dynamic caseload
