Job Description

Conducting initial assessments, developing individualized care plans, administering medications , monitoring vital signs, managing wounds, providing patient and caregiver education, communicating with physicians, and coordinating care within the patient home environment, all while ensuring the patients safety and independence at home. Summary As a Registered Nurse (RN), you will be essential in delivering high-quality patient care and ensuring optimal health outcomes. Reporting to the Nurse Manager, your responsibilities will include patient monitoring, medication administration, and developing care plans tailored to individual needs. With core skills in patient assessment and basic life support, along with premium experience in critical care and EHR systems, you will effectively manage diverse patient populations. Your relevant skills in health coaching and home care will enhance the patient experience, making you a vital member of our healthcare team committed to excellence. Qualifications Valid RN License and BLS Certification Proficient in EHR systems and medication administration Strong skills in patient assessment and monitoring Experience in critical care and developing care plans Knowledge of catheterization, IV insertion, and tube feeding Familiarity with premium skills such as Epic and Cerner systems
