Job Overview We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our team. In this pivotal role, you will provide high-quality nursing care to patients in hospice settings, ensuring their comfort and dignity during end-of-life care. You will collaborate with interdisciplinary teams to develop and implement individualized care plans, focusing on both the physical and emotional needs of patients and their families. Responsibilities Conduct comprehensive patient assessments to determine care needs and develop personalized care plans. Coordinate and manage patient care across various settings, including inpatient, outpatient, and home environments. Provide direct nursing care, including medication administration, wound care, and vital signs monitoring. Facilitate communication between patients, families, and healthcare providers to ensure continuity of care. Educate patients and families about disease processes, treatment options, and end-of-life decisions. Maintain accurate medical records in compliance with HIPAA regulations and organizational policies. Participate in discharge planning and utilization management to optimize patient outcomes. Collaborate with interdisciplinary teams to address complex medical issues such as infectious disease care, geriatrics, behavioral health, and more. Support the emotional well-being of patients and families through compassionate communication and counseling. Experience Candidates should possess a strong background in nursing with experience in various clinical settings.
Preferred qualifications include:
