Job Description

Position Summary The Hospice On-Call Registered Nurse provides after-hours and continuous on-call nursing care to hospice patients and their families during a rotating 7-on/7-off schedule. This role is responsible for triaging and responding to urgent patient needs, conducting in-home assessments, managing symptoms, and offering clinical guidance and emotional support to patients, families, and caregivers during end-of-life care. The on-call RN serves as the primary point of clinical contact outside of standard business hours, ensuring continuity of care and adherence to the patient's plan of care and hospice philosophy. Key Responsibilities On-Call Patient Care Respond to all after-hours patient and family phone calls; assess urgency and provide clinical guidance or dispatch for in-person visits as appropriate Conduct unscheduled home visits to assess changes in patient condition, manage acute symptoms, and provide hands-on nursing care Administer or coordinate medications and treatments in accordance with physician orders and the established plan of care Provide crisis intervention support and manage actively dying patients and their families with clinical competence and compassion Pronounce patient deaths in accordance with state law, agency protocol, and physician orders; notify the attending physician, medical examiner (if applicable), and funeral home Clinical Documentation Document all patient contacts, visits, and clinical decisions accurately and in a timely manner in the electronic health record (EHR) Communicate clinical findings and overnight developments to the primary care team and case managers at shift handoff Complete medication reconciliation, physician notification logs, and any required incident or change-of-condition reports Care Coordination & Communication Collaborate with the interdisciplinary team (IDT), including attending physicians, hospice medical directors, social workers, chaplains, and aides Coordinate with pharmacy, DME suppliers, and other vendors after hours to ensure patients have necessary medications and equipment Provide education and emotional support to family members and caregivers during and after patient crises Facilitate inpatient hospice or general inpatient care (GIP) transfers when symptom management cannot be achieved in the home setting Compliance & Quality Maintain compliance with federal Medicare Conditions of Participation (CoPs), state regulations, and agency policies and procedures Adhere to HIPAA regulations and maintain strict patient confidentiality at all times Participate in on-call handoff briefings, staff meetings, and required in-service trainings during the off week as scheduled Required Qualifications Active, unencumbered Registered Nurse (RN) licensure in the state of Georgia Minimum of 1-2 years of clinical nursing experience; hospice, home health, palliative care, ICU, or ED experience strongly preferred Current CPR/BLS certification Valid driver's license, reliable personal vehicle, and current auto insurance Proficiency in electronic health records (EHR) — experience with MatrixCare is preferred Strong clinical assessment skills, including end-of-life symptom management and pain control Ability to work independently and exercise sound clinical judgment in urgent and unstructured situations Excellent communication skills — written, verbal, and interpersonal Work Environment & Physical Requirements This position operates primarily in patients' homes, assisted living facilities, skilled nursing facilities, and other community settings. The RN must be able to navigate diverse home environments and respond to calls at any hour during their active 7-day on period. Physical requirements include the ability to lift up to 50 lbs, bend, kneel, and stand for extended periods. The on-call RN is expected to be within a reasonable response radius during their active week to ensure timely patient visits. Schedule Details This is a rotating 7-on/7-off position providing 24-hour on-call coverage. During the active 7-day period, the RN carries the on-call phone, responds to all patient contacts, and conducts in-home visits as clinically necessary — including nights, weekends, and holidays. The following 7 days are fully off. Handoff briefings with the outgoing and incoming on-call nurse are expected at the start and end of each rotation. Holiday coverage is shared equitably across the on-call team. Compensation & Benefits Competitive compensation commensurate with experience.
