Job Description
Help at Home is seeking a dedicated Registered Nurse to deliver compassionate in-home care for seniors and individuals with disabilities. As an RN, you will complete nursing assessments, develop and update care plans, administer medications, perform treatments, monitor changes in condition, and coordinate with caregivers, families, and providers. You will educate clients and families on health management, ensure a safe home environment, and document all services accurately. Responsibilities Provide in-home skilled nursing care to seniors and individuals with disabilities Complete comprehensive nursing assessments and reassessments Develop, implement, and update individualized care plans Administer medications and perform ordered treatments and wound care Monitor and document changes in client condition and respond appropriately Educate clients and families on disease management, medications, and safety Collaborate with caregivers, physicians, and interdisciplinary team members Ensure a safe home environment and reinforce infection control practices Maintain accurate, timely documentation in accordance with regulations and company policy Participate in ongoing training and quality improvement initiatives Required Skills Patient assessment Care planning Wound care Medication administration Electronic health records (EHR) Care coordination Patient and family education Infection control Vital signs monitoring Home safety evaluation
