Responsibilities:

Provide direct nursing care to patients in the home setting while following established care plans. Perform patient assessments, document clinical findings, and communicate updates to the care team. Educate patients and caregivers on care routines, safety, and wellness practices. Coordinate care activities to support continuity of services across the assignment.
