An opportunity is available for a skilled and compassionate Registered Nurse to deliver exceptional home health care. In this contract position, youll make a meaningful impact by providing vital nursing services and personalized attention to patients within the comfort of their homes. This role calls for a self-directed RN with strong clinical assessment skills, who excels in building rapport with patients and collaborating with cross-functional healthcare teams.

Contract Length:

13 weeks, full-time

Hours:

Monday to Friday, 8:00 AM 5:00

PM Caseload:

2532 points per week (averaging 35 patient visits per day)

Setting:

Field-based, traveling to patient homes within a designated territory

Documentation:

Home Care Home Base (HCHB); OASIS assessments

Key Responsibilities:

Complete accurate OASIS assessments and create individualized plans of care Deliver skilled nursing, including medication administration, chronic disease management, IV therapy, and wound care Evaluate patient outcomes and communicate any changes to physicians and care team members Educate patients and families on diagnosis, care plan adherence, and healthy practices Maintain timely, compliant documentation in HCHB Collaborate with therapists, social workers, and aides for integrated care Attend case conferences and participate in ongoing education as required

Required Qualifications:

Active RN license in good standing At least 1 year of recent home health (field) experience Proficiency with Home Care Home Base (HCHB) and OASIS Reliable transportation, valid drivers license, and auto insurance Strong clinical skills, autonomy, and organizational abilities

Preferred:
