Job Description

We are seeking a skilled Nurse Practitioner to provide home health risk assessments for Medicare Advantage plan members throughout Polk County, IA. This per diem opportunity offers the flexibility to design your own schedule with shifts available any day of the week, allowing you to make a meaningful impact while enjoying outstanding work-life balance.
Schedule:
Flexible shifts available Monday through Sunday Set your own schedule Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
Perform in-home health risk assessmentseach typically 45 minutes in durationin areas such as Urbandale, Johnston, Clive, and other Polk County locations Review and evaluate patients prior and current medical histories, ongoing medical conditions, and medication usage Educate patients about their medical concerns and current medications, supporting health literacy Identify gaps in care and provide well-informed recommendations to help close those gaps Visits may include annual assessments or post-discharge check-ins No requirement for treatment plan creation or prescription writing
Qualifications:
Active Nurse Practitioner license in Iowa MSN from an accredited nursing school Board certification through AANP or ANCC Valid BLS/CPR certification At least 6 months of recent nurse practitioner experience preferred This position rewards your expertise with compensation of $80-$90 per completed assessment, plus a mileage stipend and additional pay for administrative duties.
