Full-time, on-call rotation every 4th weekend About the Role Birkwood Village is hiring an experienced MDS / Care Plan Coordinator to lead resident assessments (MDS) in accordance with current rules, regulations, and RAI guidelines — a key clinical leadership role shaping quality of care across our community. Responsibilities Coordinate MDS assessments, ARDs, and timely submission; stay current on RAI Manual changes Review census changes, progress notes, and Medicare A documentation for significant changes in condition and skilled services Develop and coordinate individualized care plans, ensuring timely completion and revision with every assessment or status change Attend weekly Medicare meetings and monthly/quarterly QAPI meetings Promote patient- and family-centered care, including scheduling Care Plan Conferences Participate in on-call rotation, every 4th weekend Qualifications Current Iowa RN license in good standing MDS/RAI, PDPM, and care plan coordination experience preferred Familiarity with PointClickCare (PCC) or similar software a plus Strong organizational skills and attention to regulatory deadlines
