Job Description

Take the opportunity to provide meaningful care with a per diem advanced practice role focused on home health risk assessments throughout Lee County. As a Nurse Practitioner, you will support Medicare Advantage members by delivering key evaluations in their homes, ensuring thorough assessment and continuity of care for a diverse population.
Schedule:
Flexible shifts available Monday through Sunday Create your own schedule to fit your lifestyle Minimal weekly or bi-weekly hourly commitment, tailored to your preferences
Responsibilities:
Conduct annual and post-discharge risk assessments of Medicare Advantage plan members in various Lee County locations Perform comprehensive reviews of medical history, current conditions, and medication management during each 45-minute evaluation Educate patients regarding their health conditions and appropriate medication use Identify care gaps and deliver actionable recommendations for bridging these gaps No responsibility for creating treatment plans or prescribing medications
Qualifications:
Current Nurse Practitioner license in Iowa Valid CPR (BLS) certification required Board Certification through AANP or ANCC Minimum of 6 months experience as a Nurse Practitioner preferred Masters Degree in Nursing (MSN) from an accredited institution
