Key Responsibilities:
Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review patients medical histories, current conditions, and medications Educate patients on their health conditions and prescribed medications Identify gaps in care, providing clear recommendations to improve health outcomes Complete documentation efficiently after each 45-minute home visit No responsibility for ongoing treatment plans or prescription management
Qualifications:
Active Nurse Practitioner (NP) License in Iowa Board Certification through AANP or
ANCC MSN
degree from an accredited nursing program Current CPR/BLS certification Minimum of 6 months recent experience as a Nurse Practitioner preferred Receive competitive compensation of $80$90 per visit, as well as mileage reimbursement and payment for administrative time.
