Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. Job Description

Pioneer Valley Living and Rehabilitation is a skilled nursing facility located in Sergeant Bluff. We are looking for a new team member. The MDS Coordinator is a critical member of the health care team responsible for all aspects of the MDS process, including care planning and meeting with families regarding the care of their loved one.
Responsibilities:
Coordinate the development, completion and transmission of assessments in accordance with the regulatory requirements Review and manage reimbursement Manage care plans, including rehabilitative programs Assist with admission and discharge planning/coordination
Qualifications:
MDS and/or Restorative Nursing experience preferred Valid Iowa RN license Knowledge of clinical standards of practice, regulations and reimbursement governing long-term care and Medicare participation PVLR offers Medical, Dental , Vision, AFLAC, and 401K benefits.
