We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs within long-term care, skilled nursing, or hospital settings. Your expertise will ensure accurate completion of Minimum Data Set (MDS) assessments, facilitating optimal care planning, regulatory compliance, and reimbursement processes. Responsibilities Conduct thorough MDS assessments in accordance with federal and state regulations, including CMS guidelines and long-term care standards. Review medical records, clinical documentation, and patient histories to ensure accuracy and completeness for each assessment. Collaborate with interdisciplinary teams—including nursing, therapy, social work, and medical staff—to gather relevant data for comprehensive MDS coding. Ensure timely completion and submission of MDS assessments, accurately reflecting patient status and care needs. Maintain up-to-date knowledge of healthcare regulations such as HIPAA, Medicare requirements, NCQA standards, and long-term care policies to ensure compliance. Utilize electronic health record (EHR) systems like Point Click Care, or Epic. Support utilization review and management efforts by analyzing patient data to optimize discharge planning, case management, and reimbursement strategies. Requirements Proven experience with MDS coordination in a healthcare setting—preferably in long-term care, nursing homes, or hospitals with inpatient or ICU experience. Strong knowledge of clinical documentation standards, medical terminology, anatomy physiology, ICD-10/ICD coding (including ICD-9), CPT coding, DRG assignment, and health regulation policies. Proficiency in electronic health records management is essential. Experience with utilization review processes including Medicare and Medicaid guidelines; knowledge of CMS regulations is necessary. Background in nursing or clinical case management with acute care or critical care experience—such as ICU or Level I/II trauma center experience—is preferred. Excellent organizational skills with the ability to review complex medical records efficiently while adhering to strict deadlines. Knowledge of HIPAA compliance and state healthcare regulations related to medical documentation and long-term care standards.
