Key Responsibilities:

Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage plan members in areas such as Urbandale, Johnston, and Clive Review prior and current medical histories, existing conditions, and medication use Provide patient education focusing on medical conditions and medication management Identify care gaps and make targeted recommendations to support patient well-being and plan compliance No obligation to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Iowa Current CPR (BLS) certification Board Certification through AANP or ANCC Minimum of 6 months recent NP experience preferred Master of Science in Nursing from an accredited program
