Primary Responsibilities:

Conduct thorough annual risk assessments and post-discharge visits (approx. 45 minutes per evaluation) in members homes within Council Bluffs, Carter Lake, Glenwood, and surrounding areas Evaluate members medical histories, current health conditions, and medication use Educate patients about their medical conditions and prescribed medications Identify gaps in care and recommend practical steps for resolution to enhance health outcomes No treatment planning or medication prescribing required

Qualifications:

Current and active Nurse Practitioner license (IA) Certification in Basic Life Support (BLS) Board certification through AANP or ANCC Six months or more of Nurse Practitioner experience preferred Master of Science in Nursing (MSN) degree from an accredited program
