As a Home Health Admission Nurse at Iowa Home Care, you will play a key role in welcoming new patients and developing personalized care plans that ensure high-quality, compassionate care tailored to each individual's needs. Working closely with physicians and the healthcare team, you'll assess patient conditions and create care plans that enable them to thrive in the comfort of their own homes. Your role includes advocating for patient-centered care, educating families and support staff, and collaborating with therapists, social workers, and home health aides to ensure comprehensive support for each client's unique healthcare journey.Benefits:

Conduct thorough initial assessments of patients to establish individualized care plans in collaboration with the physician. Develop, implement, and adjust care plans based on patient needs, ensuring all aspects of care meet high clinical standards. Communicate and coordinate with families, therapists, social workers, and HHA staff to ensure a well-informed care team. Advocate for patient needs and provide ongoing education to families and caregivers, empowering them to support the patient's care effectively. Prior experience in home health or healthcare admission is preferred. Strong assessment, organizational, and communication skills.
