Key Responsibilities:
Conduct annual and post-discharge home health risk assessments for Medicare Advantage members in their residences Review and evaluate both prior and current medical histories, conditions, and medication regimens Educate patients regarding their medical conditions and prescribed medications Identify care gaps and provide recommendations to address them No need to create treatment plans or prescribe medications
Qualifications:
Active Nurse Practitioner license in IA Current CPR certification (BLS) Board Certification through either AANP or ANCC Minimum of 6 months experience as an NP preferred MSN degree from an accredited nursing school Attractive compensation includes $80-$90 per completed evaluation, along with a mileage stipend and pay for administrative time.
