Key Responsibilities:

Complete annual and post-discharge health risk assessments in patients homes Review and evaluate prior and current medical histories, medical conditions, and medication regimens Educate members regarding medical conditions and prescribed medications Identify gaps in care and provide actionable recommendations No prescriptive authority or creation of treatment plans required

Qualifications:

Active Nurse Practitioner license in ID Board Certification (AANP or ANCC) MSN degree from an accredited nursing program Current CPR (BLS) certification Six months or more of recent Nurse Practitioner experience preferred

Compensation & Additional Highlights:

You will earn $80$90 for each evaluation, with an additional mileage stipend and paid administrative time.
