Key Responsibilities:
Conduct home health risk assessments, including annual and post-discharge visits, typically lasting about 45 minutes each. Review and evaluate patients prior and current medical history, medical conditions, and medication regimens. Provide personalized education to patients regarding their health conditions and medications. Identify and document gaps in care; make clinical recommendations for resolution. No treatment plan creation or medication prescribing required.
Qualifications:
Current Nurse Practitioner license in Idaho Graduate of an accredited MSN program Board Certification via AANP or
ANCC BLS
(CPR) certification At least six months of current NP experience is preferred You will be compensated $80$90 per evaluation, with an additional mileage stipend and administrative time included.
