Key Responsibilities:
Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members Assess prior and current medical history, evaluate conditions, and review medication history Offer patient education on medical conditions and prescribed medications Spot care gaps and make recommendations for improved health outcomes Perform visits averaging 45 minutes each; visit types include annual assessments and post-discharge check-ins No duty to create treatment plans or prescribe medications
Qualifications:
Active Nurse Practitioner license for the state of ID Board Certification via AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited program A minimum of six months of current NP experience is preferred This opportunity offers competitive compensation at $80-$90 per evaluation, along with a mileage stipend and reimbursement for administrative time.
