Keystone Hospice, locally owned and located in Eagle, is currently seeking a Licensed Master of Social Work for our Weekend Social Work position. The successful candidate possesses a current LMSW license and exhibits an outgoing, professional, and compassionate presence. Our clinicians visit patients where they reside, which could include private homes, assisted living facilities, and skilled nursing facilities. Duties include but are not limited to: Provide end‑of‑life care for patients who are active, transitioning, or on daily visits Attend death visits Assist with overflow admissions and EOB's (Explanation of Benefits) Answer and respond to incoming calls and requests for assistance Communicate significant findings, changes in condition, and patient needs to the interdisciplinary team, including the Provider and Case Manager Educate patients, families, and facility staff on hospice care, symptom management, and end of life care expectations Help families with end of life arrangements Assist with pain and symptom management to promote comfort, dignity and quality of life Keystone is changing hospice care in Idaho with our exceptional clinical care, dedicated and specialized team, low clinician-to-patient ratios, and unique holistic approach.
