Job Description

Keystone Housecall, Idaho's leading housecall provider since 2013, is currently seeking a Registered Nurse for our Nurse Case Manager position. Enjoy the stability of a Monday-Friday schedule with work life balance. The successful candidate possesses a current RN license and exhibits an outgoing, professional, and compassionate presence. Our nurses contact patients both in person and via phone to quality of care, as well as establishing and maintaining relationships with hospitals, assisted living facilities, and skilled nursing facilities. This role works closely with providers and non-clinical team members to coordinate and deliver the level of care expected of our team. Keystone is changing geriatric care in Idaho with unsurpassable clinical care provided by our dedicated and specialized team. This position involves both office-based and field-based responsibilities, with many days beginning in the office and ending in the field. While in the office, responsibilities include phone triage, administering treatments, completing prior authorizations, and performing other administrative tasks. In the field, this position conducts triage visits, which involve completing focused assessments to gather information for providers to determine the most appropriate treatment plan. Depending on the patient's needs, care may include in-home treatments such as IV therapy, laboratory specimen collection, or ordering additional diagnostic tests. Patients who require a higher level of care may be referred to an acute care setting. This is a highly autonomous role that requires strong clinical judgment, critical thinking, and the ability to work independently.

  • NO ON-CALL Requirements:
  • Current Idaho RN license
  • Current driver's license, reliable transportation‪ with appropriate insurance and registration
  • Clean background check and driving record
  • One year of med/surg or geriatric experience preferred, however we will consider new graduates.
