Job Description

Deliver exceptional patient care and make a meaningful difference as a Nurse Practitioner providing home health risk assessments on a per diem basis. This role involves visiting Medicare Advantage plan members in Jerome Countyincluding the communities of Jerome, Hazelton, Eden, and Grangevilleoffering valuable annual and post-discharge health assessments directly in the comfort of their homes.
Schedule:
Flexible shifts available Monday through Sunday Design your own work schedule Commitment is based on minimum weekly or bi-weekly hours
Role Overview:
Complete comprehensive annual home health risk assessments for Medicare Advantage members Review and evaluate prior and current medical history, chronic conditions, and medications for each patient Educate members about their individual health conditions and proper medication use Identify care gaps and provide guidance or recommendations for resolution No direct treatment planning or prescribing responsibilities
Requirements:
Current Nurse Practitioner license in the state of Idaho Active CPR certification (BLS) Board Certified through AANP or ANCC Minimum 6 months Nurse Practitioner experience preferred MSN from an accredited nursing school Your dedication will be rewarded with $80$90 per evaluation, as well as compensation for mileage and administrative time.
