Make a meaningful impact in the lives of Medicare Advantage plan members by conducting home health risk assessments as a Nurse Practitioner. This per diem opportunity offers you the flexibility to create your own schedule with shifts available throughout the week, including weekends. Youll visit individuals residing throughout Jerome County, including Jerome, Hazelton, and Eden, supporting each patient with comprehensive annual or post-discharge assessments in their homes.

Key Responsibilities:
Perform detailed home health risk assessments for Medicare Advantage plan members Review and evaluate each patients medical history, current health conditions, and medication regimens Deliver education regarding medical conditions, medications, and promote overall wellness Identify gaps in patient care and suggest solutions to improve patient outcomes No prescribing or treatment plan development required, focusing solely on assessment and education
Desired Qualifications:
Active Nurse Practitioner License in Idaho Board Certification through AANP or
ANCC MSN
degree from an accredited institution Current CPR certification (BLS) At least 6 months of recent Nurse Practitioner experience preferred
