Key Responsibilities:
Perform annual home health risk assessments and post-discharge visits in patient homes across Kellogg, Pinehurst, Osburn, and surrounding areas Review patient medical history, current conditions, and medications Educate patients on health conditions and their medications, providing valuable insight and support Identify gaps in care and recommend appropriate steps to address those needs Focus entirely on assessmentsno prescribing or treatment plans required
Qualifications:
Active Nurse Practitioner License in the state of Idaho Current CPR certification (BLS) Board Certified through AANP or ANCC At least six months of recent NP experience is preferred Master of Science in Nursing (MSN) from an accredited program Your commitment to high-quality care makes a real impact on community health outcomes.
