Job Description

Take on a dynamic per diem role supporting Medicare Advantage members with comprehensive, in-home health risk assessments in Jerome County, ID. As a Nurse Practitioner working in the home health setting, youll conduct focused evaluations that play a critical role in shaping ongoing care. This opportunity features autonomy in scheduling and the ability to make a direct impact in multiple communities, including Jerome, Hazelton, Eden, and surrounding areas.
Schedule:
Flexible shifts available Monday through Sunday Self-scheduling for optimal work-life balance Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
Complete annual home health risk assessments for Medicare Advantage plan populations Review and evaluate past and current medical history, diagnoses, and medication regimens Deliver patient education concerning health conditions and prescribed treatments Identify care gaps and provide actionable recommendations for improved outcomes No need to create ongoing treatment plans or prescribe medicationsfocus is on assessment
Qualifications:
Active, unrestricted Nurse Practitioner license in Idaho Current CPR certification (BLS) National Board Certification (AANP or ANCC) Minimum MSN from an accredited nursing program At least six months of recent Nurse Practitioner experience preferred This position provides $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
