Job Description

Deliver essential healthcare directly to patients by conducting home health risk assessments in Elmore County. This per diem opportunity empowers you to manage your own schedule, with flexible shifts available any day of the week. Youll serve Medicare Advantage plan members in communities such as Mountain Home and Glenns Ferry, ensuring that each patient receives a thorough evaluation right in their home.
Schedule:
Flexible shifts available Monday through Sunday Design your own work schedule Meet a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Perform comprehensive annual home health risk assessments for Medicare Advantage plan members Review and evaluate prior and current medical history, ongoing health conditions, and medication use Educate patients about their medical conditions and prescribed medications Identify and address any gaps in care, offering actionable recommendations No requirement for treatment plan creation or prescription writing
Qualifications:
Active Nurse Practitioner license in Idaho Current CPR certification (BLS) Board Certification via AANP or
ANCC MSN
degree from an accredited nursing program At least 6 months of experience as a Nurse Practitioner preferred This position offers competitive compensation of $80$90 per completed evaluation, along with mileage stipend and reimbursement for administrative documentation time.
