Key Responsibilities:
Complete annual home health risk assessments for Medicare Advantage plan members throughout Payette County, including Payette, Fruitland, and New Plymouth Evaluate each patient's current and previous medical history, underlying conditions, and medication usage Educate patients on managing their health conditions and understanding their medications Identify care gaps and recommend next steps to support improved health outcomes No requirement for development of treatment plans or prescription of medications
Qualifications:
Valid Nurse Practitioner license issued by the State of Idaho Board Certification (AANP or ANCC) Up-to-date CPR (BLS) certification MSN completed at an accredited nursing school Minimum six months recent nurse practitioner experience strongly preferred Youll receive $80$90 for each completed evaluation, along with a mileage stipend and compensation for administrative timereflecting the expertise you bring and your dedication to quality care.
